Create a Customized Checklist/Submit Walk Species Lists
Use this form to create a printable list, and optionally, to email it to yourself or others
After a walk, create a list of the mushrooms you have seen. You can then send this list to the club
recorder for posting on our web site. You can also print a copy of the list or email it to
yourself or anyone else. (The email is standard ascii text which you can copy and paste into other
applications.) You can enter your list using either scientific or common names, or some of each. If you
cannot find a particular species, you can type it into the "Notes" box and we'll include it in an updated
version of this form.
The following table is based on the WPMC
life list augmented with the scientific names
of additional species listed in the common name table below.
